Cheek (Dimple) Piercings in Calgary
High-commitment facial piercings placed through the cheeks or dimples, done with careful anatomy checks, long-term planning, and an honest conversation about scars and oral health.
What are cheek or dimple piercings?
Cheek piercings, often called dimple piercings, pass through the tissue of the cheek on one or both sides of the face. When placed over natural dimples, they can accent them; when placed on flat cheeks, they can create the appearance of dimples when you smile.
These piercings use extra-long labret posts during the initial swelling phase, with flat discs inside the mouth and decorative ends on the outside. Over time, if your anatomy and habits allow, the posts are downsized to shorter, more comfortable lengths—but cheek piercings always remain higher-maintenance than most other facial piercings.
At Tribal Expression in Calgary, we treat cheek piercings as advanced, high-commitment piercings. They can be beautiful and expressive, but they also come with permanent dimpling, visible scars, and potential effects on teeth and salivary glands. We’ll walk you through all of that before you decide.
Are cheek piercings right for me?
Cheek piercings are not “starter” piercings. They’re best suited for clients who:
- Already have experience healing facial or oral piercings.
- Understand that scars and dimples are permanent, even if the jewelry is removed.
- Are prepared for a long and sometimes bumpy healing process.
- Can commit to regular check-ins and downsizing appointments.
During your consultation, we’ll look closely at:
- Cheek thickness and placement over teeth – Ensures we’re not placing the disc where it constantly hits enamel.
- Smile line and facial movement – The cheeks move a lot; we want to see what happens when you talk and laugh.
- Location of the parotid duct (Stensen’s duct) – This duct carries saliva from the parotid gland into the mouth. We avoid placements that could irritate or damage it.
- Existing dimples, scars, or dental work – These change how the piercing sits and heals.
- Healing history and lifestyle – Smoking, grinding teeth, clenching, or chronic inflammation can all affect results.
In some cases, the safest and most ethical recommendation is not to pierce the cheeks at all. If that’s the case for you, we’ll explain why and suggest safer facial piercing options instead.
How much do cheek piercings hurt?
Cheeks are a fleshy, expressive area, so you’ll definitely feel these—but the piercing itself is still quick and controlled. Most clients describe:
- A firm, deep pinch as the needle passes through the cheek.
- Warmth or pressure that fades shortly afterward.
- Moderate swelling for several days, especially when eating or talking.
We’ll support the tissue properly, coach you through breathing, and make sure you’re positioned comfortably. The more important “feeling” with cheek piercings is often the first week of swelling and getting used to longer jewelry, rather than the moment of piercing itself.
Healing time for cheek / dimple piercings
Cheeks are one of the slowest and most demanding piercings to heal. It’s important to be realistic:
- Initial healing: commonly 3–6 months.
- Full internal healing: often 6–12+ months.
Typical healing pattern:
- Days 1–7: Significant swelling, extra drooling, and awkwardness when eating and talking.
- Weeks 2–6: Swelling starts to stabilize; we begin planning and scheduling downsizing.
- Months 2–6: Ongoing fine-tuning of post length and jewelry fit to protect teeth and gums.
- Months 6–12: The piercing slowly becomes less reactive, though flare-ups can still happen if bumped or irritated.
Even once healed, cheeks remain high maintenance. They can be prone to swelling during illness, nighttime teeth grinding, or changes in weight. They also tend to flare if the jewelry is too long, too short, or not adjusted as your face changes over time.
Cheek piercing aftercare
Cheek piercings require a mix of external skin care and gentle oral care, plus careful attention to swelling and jewelry length.
Outside care – skin on the cheeks
- Use our recommended hypochlorous acid aftercare spray 1–2 times a day (or as directed).
- Spray the area so the solution covers the entire entry around the post.
- Let it sit for 30–60 seconds to loosen any dried buildup.
- Gently pat dry with a clean, disposable paper towel—no rubbing, twisting, or “spinning.”
Inside care – mouth, teeth, and ducts
- Rinse gently with plain water after eating to reduce trapped food around discs and posts.
- If needed, use a mild, alcohol-free mouth rinse once or twice daily.
- Avoid strong antiseptic or whitening mouthwashes that can dry and irritate tissue.
- Brush your teeth as normal but move slowly around the inside discs.
If you notice constant wetness from one cheek, thick saliva, or unusual swelling near the back of the cheek, let us know—those can be signs of irritation near the parotid duct and should be checked promptly.
Swelling management & habits
- Stick to softer foods at first and take small bites.
- Avoid chewing on the posts or discs—this can damage teeth and stress the piercing.
- Be mindful of nighttime teeth grinding or clenching; in some cases we may suggest discussing this with your dentist.
- Try to sleep on your back or opposite side while swelling is present.
Downsizing is not optional with cheek piercings—it’s part of what keeps them safe. We will recommend follow-up visits to gradually shorten the posts as the cheeks settle.
What jewelry is used for cheek piercings?
Cheek piercings are almost always done with long, flat-back labret posts made from high-quality implant-grade materials:
-
Initial jewelry
Extra-long posts with flat discs inside the mouth to allow for major swelling without embedding. -
Implant-grade materials
ASTM F-136 titanium manufactured and finished to medical standards, with the option to upgrade to 14k or 18k solid gold tops once healing allows. -
Threadless or internally threaded ends
Smooth through the tissue and more stable for long-term wear. -
Progressive downsizing
We shorten the posts over time, protecting teeth and gums and reducing movement through the piercing channel.
Because Tribal Expression manufactures implant-grade jewelry in-house in Calgary, we can fine-tune post length, disc size, and decorative ends specifically for your anatomy and how your cheeks move when you talk and smile.
Risks of cheek piercings (and how we manage them)
Cheek piercings come with more potential complications than many other facial piercings. These may include:
- Permanent dimples and visible scars even after jewelry removal.
- Enamel wear or chipping if discs hit teeth repeatedly.
- Gum irritation or recession if jewelry is too long or sits in the wrong place.
- Issues near the parotid duct, such as altered saliva flow, if placement is not careful.
- Prolonged swelling or drainage compared to other facial piercings.
We minimize these risks by:
- Being selective about who we pierce and where we place the jewelry.
- Using only high-quality, implant-grade, well-machined jewelry.
- Building a downsize plan into your healing from day one.
- Encouraging regular check-ins and honest communication if anything changes.
If at any point the risks begin to outweigh the benefits—for example, if we see ongoing dental or gum issues—we’ll talk about safe retirement and scar management options.

Placement & marking
We mark potential sites and have you check them in relaxed expression and when smiling so you can see how they line up. -

Cheek Piercing FAQ
How long do cheek piercings take to heal?
Cheek piercings are slow healers. Expect several months of active healing, and often 6–12 months or more for full internal stabilization. Downsizing jewelry along the way is essential to keep them as healthy as possible.
Do cheek piercings leave permanent dimples or scars?
Yes. Cheek or dimple piercings almost always leave visible dimples and scars, even after the jewelry is removed. For some people this is part of the appeal; for others it’s a reason to choose a different facial piercing.
Can cheek piercings damage my teeth or gums?
They can if the discs hit your teeth or rest against your gums, especially if the jewelry is too long, too short, or not adjusted as your cheeks change over time. Working with experienced piercers, using high-quality jewelry, and following downsizing recommendations all help reduce this risk.

Here at Tribal Expression, we implement the highest possible sterilization standards in Canada, the same standards you would see at a plastic surgeon’s office. We feel this is only right as any invasive procedure can be life-threatening if proper sterilization protocols are not met.
We ask that friends and family wait at a nearby coffee shop or in their car during the procedure. Please eat a light snack 30 minutes before your appointment.
